To be correct, it is a Statutory Declaration which carries legal weight in itself, so to which party (Authority or Schools) is an immaterial point. It's not something to be taken lightly.
On the question of whether the whole issue could be prevented by system, it's a fine balance to achieve between privacy and convenience. You can't have both ways. I personally don't want the government to know everything automatically. I suspect what happen was correct. They let the parents declare, but did offline verification, which detected the discrepancy.
